We are in-network with:
- Blue Shield of California Commercial PPO
- Medicare for the purposes of diagnostic testing, provided that the testing is ordered by your physician and the services are medically necessary.
- United Healthcare Commercial PPO
- United Healthcare Medicare Advantage PPO plans.
Please understand that we are not providers for United Healthcare Hearing Network (UHCH), which is separate from the health care plans.
We are not contracted with any other plans. We will gladly submit the insurance claims on your behalf; however, payment is due in full at the time services are rendered.
Good Faith Estimates
Notice to clients and prospective patients: Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including Audiology services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including Audiology services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
